Onsen-san is a recurring character from the original game.
It sits on the road in the Road to Forest, blocking Madotsuki's way. Killing it with the Knife will result in it seemingly transforming into Shitai-san. Madotsuki is forced to wake up, disturbed.
If you've yet to kill him, you can spot him through the window of the Hot Spring House in the Infinite Wilderness.
